Form A - Formal Application for Certification
Form A requires you to provide information regarding school size, certification level desired, contact information, and application submittal and completion dates. Please fill out all required fields before submitting. A $500 fee is also required as a part of this form's submission. Please name this form in the following format: "InstitutionName_FormA" (e.g., if ASU, "ASU_FormA").

Form B - Document Cover Page
Form B requires you to provide information regarding school name, point name, and point number, and to attach the form to any file upload for a given point. Please fill out all required fields before submitting. Please name this form in the following format: "InstitutionName_PointNumber" (e.g., if ASU, "ASU_1.1.1").

Form C - Certification Submission Cover Page
Form C requires you to verify that all documentation that is required to meet the desired certification level has been uploaded, and also asks you to specify how many points you plan to earn. Form D - Appeal Request Form
Form D requires you to provide all information regarding school name, point name, point number, and an explanation on why BEEnow should reconsider accepting the documentation uploaded for a particular point. Form E - Sign-Off Letter from School Head or Chairperson [Template]
